Your contribution of $15 per necklace is a tax-deductible contribution to the Jennifer Linscott Tietgen Family Foundation.  The JLT Foundation is a non-profit 501(c)(3) organization.

Upon submission of the following order form you will promptly receive confirmation of your order along with instructions of where to send your check, made out to the JLT Family Foundation.

Once your check has been received, your necklace will be mailed directly to you through First Class Mail.
